Bicycle Registration Form
The Urbana Police Division recovers numerous bicyles throughout the year that are never able to be returned to their rightful owners.
The Urbana Police Division provides a service to its citizens to help return these bikes to their owners. Registering your bicycle takes only a few minutes.

- Or you can also deliver this registration form to any Urbana Police Officer that you see on patrol.

Citizen Commendation
The Urbana Police Division welcomes citizens' recognition of police employees performing in a quality manner.
Citizen commendations will be forwarded to both the commended employee and to his or her personnel file.
Citizen Complaint Form
The Division and it's employees, are committed to maintaining integrity, discipline, and a relationship with the community of public trust and confidence.
The Division has a "Professional Responsibility" process to accept and investigate allegations of employee misconduct.
The Professional Responsibility process requires that all complaints be accepted, investigated, and then finished with a "conclusion of fact."
If appropriate, employees will be disciplined for misconduct according to the Division's disciplinary procedures.

Statement Form
One day, you may become involved in an incident within the City of Urbana and the police division will request you to fill out a statement form.
A written statement is important as it forms part of the evidence the police are collecting.
The statement may be used to help decide whether a crime has been committed. Does the statement a witness provides give enough detail to warrant a criminal complaint?
A written statement protects everyone that is involved. It is your statement! It cannot be changed. When a statement is given orally, thnigs can be added or ommitted. In a written statement, nothing can be changed unless the person giving the statement is aware of it and agrees with those changes.

Remember!! a statement form is a legal document! You can be prosecuted for knowingly making a false statement.
When filling out the statement form, try to use plain english. Be very detailed by using dates and times, person's names, etc. Follow the instructions on the form.

- According to the Ohio Historical Society, in 1840 during the VanBuren-Harrison contest, downtown Urbana was the site for a national Whig convention dinner. Hanging nearby was a banner with the words "The People is Oll Korrect."
